Thank you for your company/organization’s participation in the Electric Bicycle Loan Program. This document defines the agreement between you the (applicant/company/organization) and the Association of Monterey Bay Area Governments, (AMBAG). This program provides no fee leases of electric powered bicycles to businesses and public agencies. The bicycles are the property of the AMBAG and are leased only for the maximum period noted above..
We understand that this is a commuter demonstration project for businesses in Monterey County and government agencies in Monterey, San Benito, and Santa Cruz Counties. We understand that we must execute this contract and a liability waiver form, and attend a training class in order to qualify our company/organization's participation in this program..
As a participant in the Electric Bicycle Loan Program, we also hereby acknowledge the following:

Any employee who rides the electric bicycle at our company/organization will wear a D.O.T. approved bicycle helmet when riding the electric bicycle at all times. I also agree to have every rider sign a waiver and complete the appropriate rider’s surveys.
Our company/organization has made a voluntary request to ride the electric bicycle and holds AMBAG (all employees, officials, agents, representatives, and sureties) harmless from any liability that we assume when we sign the liablity waiver form.
Our company/organization has been loaned a separate electric bicycle charger unit which will be returned with the electric bicycle on the above expiration date.
Our company/organization has liability and theft insurance that covers my employees while using the electric bicycle.
Our company/organization is liable for any damage to the bicycle beyond normal wear and tear. (initial:          )

We have read the foregoing paragraphs and fully understand the terms contained therein and sign this lease agreement freely and without inducement.
